Entering a survey

Although each survey is different almost all questions asked are one of the 8 different versions discussed here. These are divided in three segments.

In the first segment basic questions are discussed:

1) Closed question with a single choice (e.g. what is your gender?)
2) Open question asking for a number (e.g. what is your age?)
3) Open question asking for text (e.g. what is your name?)

In the second segment a few trickier type of questions:

4) Semi-closed question with a single choice (e.g. an option 'please specify____' is added)
5) Closed question with multiple choices (e.g. which of the following apply?)
6) Semi-closed question with multiple choices (combination of the previous two)

In the third segment two other type of questions that might also occur:

7) Ranking question (e.g. please rank the following four items)
8) Pick X from Y (e.g. please choose two options from the ten listed below)
